If the goal is simply to get accepted to a US medical school, I think you should be ok, but if you wish to target top schools or merit aid you should consider a gap year to strengthen your application. In either case apply broadly.
You really need to apply to the places that match your stats. You are unlikely to receive interviews where your MCAT is above the 90th percentile (except your state schools).
You could look into something like a crisis hotline or something. LizzyM's said that can be called clinical - that it fits into a gray area. Keep doing that for the rest of this cycle.
