One thing you’ll learn over the course of your career is you have to look out for your own interests. Your employer will, without fail, look out for their own interests as well. It is in your interest to cultivate job offers, especially since 60% of applicants to medical school are rejected each year. Good luck with your application cycle and the job search.
 
As someone who was in a similar position, I'd say take the job. You don't know what may change over the course of the next year and there's some good insight that can be gained regardless.

However, don't sign any contract that binds you to work. If you the job offer OP before any MD acceptances, they may require you to make a decision in a month or two. You could also renege although not sure how that'll look contractually.
 
Companies can’t compel persons to work for them regardless of what the contract says. Even the notice requirements aren’t written in stone. If there is a signing bonus, they can and will claw that back but beyond that there isn’t much they can do. The only thing that could come into play is a non compete clause but that really wouldn’t apply here. OP is free to line up offers from however many prospective employers as they desire.
