Just out of curiosity. Signing a LOI with a prematch program is only done with programs that are not involved in the main match? How does one go about doing this? Networking or being chummy with a home program's PD?
 
If you didn't submit a rank list, then there is no harm done.

If you did submit a rank list, then first of all I'm really disappointed. You should have withdrawn the minute you took a spot. IN fact, the program that took you should have required it as part of accepting the spot. If you fail to match to any spot, then again no bad outcome for you or anyone else. If you do match to a spot, then someone else lost that spot because of you. You knew the rules going into this, it was your responsibility to deal with it.

If you match to a spot, you will not get "a waiver". You'll be assessed a match violation. What happens next depends on lots of factors:

The NRMP will likely give you a match ban. It could be for 3-5 years, or lifetime. Depending on what field you're going into, this could limit your ability to get fellowships or any further training via the match.

You could try to resign your prematch spot and keep your match spot. If you try to do that, the pre-match program may report you to the NRMP. IN that case, you'll probably lose both spots.

Your prematch program may, or may not, care about this. They might consider it unprofessional behavior. They might void your spot over it, o rthey might not. If other programs are part of the NRMP match at your institution, the NRMP might bring pressure on your program to address it.

And the NRMP could report this to the state Board where you'll be working. It's possible that this could have an adverse effect on your licensing there.

I am hoping your situation is that you didn't withdraw but didn't submit a list. Again, in that case, no problem.
