Goodness, yes, neurotic. Pick a single geographic location you want to live in and cut out the rest. You're competitive. Personally, I'd just stick with the CA programs, but then I'm kind of biased. I can't do snow (so no Arrowhead for me) or humidity. There's a very wide range of competitiveness in CA programs already and you have a clear reason for wanting to be in CA. Geography is, by far, the most important factor. You're competitive. You need to decide WHERE you want to live. Remember, unlike med school, the vast majority of people stay around where they completed residency. You're, statistically, deciding where you want to live the rest of your life, not primarily deciding where you want to do four years of training.
