I'm a 2025 graduate (so 2+ gap years) putting together my application for the upcoming cycle. I pivoted late from PhD (I really do want to focus on the clinical side of things, so I've decided against MD-PhD), so I'm aware that I'm quite lacking in volunteering and shadowing. I'm actively working on both but balancing with work has been tough. I'm mostly looking for help with my school list; I'm kind of lost, as my stats theoretically qualify me for T20s but I'm fully aware that the rest of the app is pretty thin. I know the current list is very top heavy, so I'm looking to narrow it down a bit and find more target schools that are comfortable with high-stat low-service. More details:

• cGPA: 3.89, sGPA: 3.89
• MCAT: 526 (132/130/132/132)
• Residency: Massachusetts resident but haven't lived here long enough to qualify as resident, though I spent a year in Boston before college and plan to be here until starting medical school. I have Oregon "heritage" status for OHSU.
• ORM (white)
• T5 undergrad school
• Clinical: ~1600 hrs (paid) as a full-time anesthesia technician in a major hospital, working closely with physicians in the OR. No clinical volunteering at the moment, though I'm starting in the emergency room next week at a local community hospital and should get a good amount of hours over the next couple of months.
• Research: Three summers full-time and part time during the school year in college (~1500 hours), plus full-time research in an MIT lab for the past 8 months. Honors thesis/presentation, poster presentations following each summer, third-author publication, but no first-author. I'll probably be at least second author on at least one preprint before the end of the year.
• Shadowing: Currently only 20 hrs split between anesthesia, internal medicine (GI and liver), and rheumatology. I'm hoping to get to 40 before submitting. I also had a lot of opportunities during the anesthesia job to basically unofficially shadow anesthetists, which I often did and got a lot out of.
• Non-clinical volunteering: ~50 hours volunteer tutoring for underserved kids (started in October 2025, will end with ~70 hrs)
• Activities: 3 years in a club-level sport in college, currently playing IM basketball at MIT, 2 years in my school's pre-med association including 1 year on exec board (I wasn't very passionate about it though)
• Awards: nothing of note, though graduated with honors.
• LORs: postgrad PI, undergrad PI, undergrad physiology professor, undergrad literature program director, tutoring coordinator, rugby coach, undergrad neuroscience professor (probably a bit weaker)
• Misc: I took a LOA for a quarter during my senior year to deal with chronic illness, which ended up being a good thing (and motivated to full commitment to MD) but I realize it can have some effect.

When did you take your MCAT?

You need 250 hours of service orientation activities to keep up with applicants vying for a seat at a brand-name program. Tutoring, teaching, or mentoring will not suffice for this. Food distribution, shelter volunteer, job/tax preparation, legal support, transportation services, or housing rehabilitation. You need to make a clear(er) break towards clinical patient-focused care. I don't quite see it in this profile yet.

As noted above, your lack of non clinical volunteering will limit your chances for interviews at top tier schools. You could add these schools to your application:

Like I said, I fully understand that I’m not where I should be with volunteering, but I can sincerely say that I’ve made an effort to do as much as I can. I appreciate your advice about diversifying and will look into it going forward—hopefully the ER is a good place to start. I do intend to apply this cycle though, so do you have any advice regarding schools or strategy?

The internet is full of videos of 520+/3.9 applicants who failed to get medical school acceptances. Every cycle will cost you at least $2500, time, and emotional stability; make it count by submitting your best-possible application. I've given you my 250 hours advice; strategy relies on your mission fit. Just a fair warning that you shouldn't drive a car with three wheels in an F1/NASCAR race.
