Step II 2nd-line Antibiotics

I thought it would be good to create a running list of diseases where the second-line antibiotics should be known. This sometimes comes up in questions as a patient with an allergy to the first-line drug or a contraindication to the first-line drug. I'll start:

Lyme's disease - 1st-line doxycycline; alternative is amoxicillin or cefuroxime
