I have 2 from neurology (1 chair) and 1 nephrology...should I get a 4th?

It wouldn't hurt to have 3 neuro ones, you can use the neph one for prelims if you want. But if you think the neph one is an outstanding letter, I suppose you're good to go!
 
Two neuro and one medicine should be fine for most programs. Look at each program's website for their requirements. My anecdote is that I applied with two neurology and one medicine and had no problems.
 
Sorry to not comment on this earlier - been a bit busy.

I love letters from neurologists who have worked with the applicant. They will definitely be seen as stronger in support of your ability to be a neurologist.

If you have one from a non-neurologist, it is not terrible so long as it is a really strong, glowing endorsement - internal medicine ones are probably next best for this.

Three letters are fine - pick your best three and use those - lukewarm letters can hurt you.
