3 or 4 letters for residencies?

3 is fine. You can include the 4th if it's strong, but don't expect it to give you much additional benefit.
 
any suggested breakdown for the letters? Like should I try and get 3 DR/IR letters or other specialties too?

The consensus I've found to be true is the golden zone is 2 DR and 2 clinical (medicine, surgery, FM, etc). There's a lot more good stuff to be said about you by a non-rad that has worked with you unless you're rad letter is IR or a faculty you did research with
