You quit the harder job. Everyone is mobile these days. Yes it can be a burden one family’s. But it’s far better to work away and work less and make twice as much away and be home home after making the cash. Because ur presence at home and more time off is actually better and ur spouse will actually appreciate you home for 26 weeks out of a year. Fully at home and not even working.

A few of the professionals locums said it actually saved their marriages to work away from home. Because they were miserable with the daily grind coming home 6pm grumpy. Now they have more time fully attentive to the family at home since their time off is massive. And their overall pay is the same or even more than their daily grind job working 44 weeks a year.
We are in the same group. I have to eat ****. I will do the least amount of work possible. Cancel cases, say no to add ons, find reasons no to do epidurals, blood patches, central lines., etc. That's my new incentive. Disparage the docs/leadership that make this choice.

And multi sites practice “same job” do encounter this often like you say. Quit. Move on.

Stay and do as little as possible not to get fired and lie in the tall grass for the people who handed me this **** sandwich.
I switched 3 full time w2 jobs the last 4 years. If a job isn’t working. Leave. Especially if you are in an urban area.

Jobs are not marriages where u stay or try to stay in for the long haul. Jobs are like situationships where there is now long term defined commitment. We are all as good as 60-90-120 days notice are 99% employed jobs. Almost any employer can get rid of u outside of federal employment jobs after 2 years probation period.

Doc Y: Dude, you are spending most nights at your site in the call room while I am lucky if I see the call room in my hospital. I should be paid more per hour

This is not an uncommon scenario.

The person taking the “harder” call should absolutely be compensated at a higher rate but I never support a home call back up that never gets called getting zero dollars.

Anyone who is on call should get “something”.

We are private, so we control how the money is divided up, which may not be the case for you. But what we do is stipend the calls (modestly) so that people are willing to take them, but everybody still collects units for the actual work they do. So if one person has a busy night on OB, and the other person slept all night, the busy person makes a bunch more money.

You could triple my salary and still not be able to convince me that spending half the year away from my family is a good thing.

Of you could be in a miserable job 15-20 min from your house and come home cranky most days and that affects your family more. Stuck late many days. Bad work environment carries to your home because you come home in a bad mood most days. Coming home moody daily and barely being “present” mentally for the family may not be a good thing either.
 
I live in central Florida from Tampa to Daytona. So know the entire area well

It’s always a cat and mouse game as hospitals take over w2 in house.

Most of the base salaries range from 450-525k in this region with 9-10 weeks off. Probably 1/3 of the practices have already gone to some sort of compressed schedules 20-26 weeks off already in the area. Many versions of the compressed schedule depending on facilities needs.
Uneven distribution (nightfloat with by far the better deal leaving w2 daytime staff working regular days 5 days a week).

Some days are shifting to longer 12 hr days time and 26 weeks off. But they are forcing daytime to do at least one full weekend day. That’s brutal in my opinion

There are practices that do 73 calls a year 24hr. There are practices where everyone participates in nightfloat plus days. (I think that’s the fairest system even distribution of work load).

There are “light” call back hospitals where the doc is on 168 hrs beeper every other week and one week off . (Those places seem to have the highest turnover of docs leaving)

There is excellent place where I live where the docs can’t agree on a model and stuck with the old blockbuster video management model q 5/6 calls. Because the older docs “love” their pre and post call days off and coming in at 5pm for call. But they they are wanting more money (like everyone else). They all get 10 weeks off. I analyzed their staffing and said they can easily go to 18 weeks off with even night float distribution (beeper with a crna in house and another crna on beeper for the or) no trauma. 8 weeks extra off is easily worth 150k “pay raise” with very little extra work involved.

Not sure if it’s that black and white. You can also be miserable far away from home. I’m happy with my lowly W-2 job 20 mins from home. Reasonable hours/pay/time off, great work culture/collegial surgeons, come home in a great mood.

Do I trade that for more money and 26 weeks away from family? Nope.

Where I am located, the salaries are all in the 450k range. That can either be a job with the typical 8-10 weeks off, one post call day a week, some earlier days, one weekend a month typical job. Or it can be a job where you have 13 or 17 weeks off but don’t have any early or post call days, working regular days Mon to Fri. Plus all the drama of working for a bad anesthesia group and hospital system. Constantly moving goalposts. Bad leadership. Etc. and coming home after a 10 hour day and being fully present and engaged with another day of work the next day is not so easy.

Or I can go away for 7-10 days a month and make the same money or even more and not deal with all the other headaches plus the benefits of 1099 pay. I can have flexibility and choose my weeks. And be completely free and present for 35+ weeks of the year.

Some w2 jobs are great with pay and benefits and hours and leadership but some of us are not so lucky and have to find the lesser evil

Florida everywhere is pretty bad unless you find pockets of good jobs with lots of time off

South is probably the worst overall area

No matter how you structure it someone will think it’s unfair.

If everyone covers every call, I think the fairest way to do it is to assign them evenly and let the market dictate the stipends (I.e. raise them until each call can be traded easily).

If not everyone can cover every call, it’s a bit more complicated. I think you have to look at the data: How long are the calls? How efficient? Middle of night call backs? Acuity? Subspecialty requirements? The more painful calls should have higher stipends. This is still usually reflected in the market (I.e. how easy is it to trade out of a given call?), but the extra data can help justify higher stipends to the people who only cover the easier calls.

Again, the people who get a lower stipend will still complain about it not being fair, but it’s harder for them to argue with the data.

“These are bars” as the kids would say and people should keep this in mind when job hunting. Sadly it’s hard to tell until you’ve been in the game a bit and made a couple errors but a fat check isn’t always the answer to happiness…..though it can help
