I am asking this for my sister. I bolded couple of damaging factors. She is also adding multiple OOS friendly schools.
Status: Permanent Resident, Recently became a Texas Resident. (was previously a FL resident) Finished all TX secondaries in late August to Early September.
GPA: 3.92 with Biology major and Math Minor from a TOP 10 State University(Not in Texas) with Full Ride Scholarship for four years
MCAT: 512, third time, Verbal is 127 (would be 10 in old mcat) I figured out how to manage my time. My previous attempts were due to my poor timing skills.
Previous MCATs
(1) 31 with 7 on verbal and 12s on each science
(2) 503 with 122 on verbal (would be 5 in old mcat)
Medical Volunteering: just typical, hospice, hospitals, homeless clinic, mission trip
Non-medical volunteering: big bro big sis, soup kitchen, Sunday school
DAT(Dental Admission Test): 28 (99.9 percentile, Harvard Dental Average is 24(99 percentile) and 3.85 GPA) (In case MD does not work out)
Research:
(1) Home University, two scholarship programs, Senior thesis
(2) University of Michigan School of Medicine, under the Chair of Physiology (MD): Second Author Publication in a middle tier journal
(3) National Cancer Institute, under the distinguished investigator (MD) (top 2~3% among all 1,000 NIH PIs, two students of his had Nobel Prizes): Five Co-Author papers with double digit impact factors
(4) MD Anderson, Baylor College of Medicine: One of top MD investigators in Baylor and MD Anderson: Two Co-Author papers coming soon
Letters:
(1) Strong letter from one of Top MD Principal Investigator in MD Anderson And Baylor College of Medicine with $15 million NIH Grant
(2) Strong letter from the MD Chair of Physiology department in the University of Michigan Medical School
(3) Decent letters from sciences and non-science professors from her top 10 state university
(4) A moderate letter from the MD Distinguished Investigator at National Cancer Institute. Did not use it. Despite having 5 co-author papers in middle-tier and decent journals, the PI specifically stated that it is "moderate" since the postdoc in his lab did not like my sister. He does ask whether my sister uses his letter or not.

Not applying to MD PhD programs due to not having high MCAT scores
But applying as a future physician scientist with MD only with a plan to do a research focused primary care residency and then research focused subspecialty fellowship programs to establish her niche and find an academic job in medicine
Do you think she should apply to FL schools because she grew up in FL and can switch to FL residency in a very short period of time?
Do you think she could be accepted? any possible interviews?
