62311/77003 coding

Just reviewing my handy CPT book, noticed that 62311 excludes 77003 unless a formal contrast study is performed. Are all of you using contrast? What counts as a "formal contrast study"?

I found it in my INGENIX CPT 2008 code book...

It excludes epidurography (77275), NOT fluoroscopic guidance (77003), without a formal contrast study.

For those still billing 77275 and trying to keep it clean, what exactly are the essential components of a formal epidurography report?
