Ok, so I am trying to catch more of these and know you need at least 2 ROS. But what exactly suffices as 2 ROS? If you say energy (general) is decent and there is no nausea (GI), does that count? Or do you need to ask more? For example: no fatigue, no weight loss AND no nausea and no diarrhea?
 
Is there a reason you need the history elements for the 99214? You need 2 of the 3 categories. I know MDM isn't required, but without it justifying the 99214, it seems hard to say that the extra history and exam you do is medically necessary. So with the MDM covered, I always have enough of an exam to meet the 99214 standard and never look at the history. It seems difficult to me to not have enough exam elements.

Ah, you are totally right! I completely forgot about it. I'll just blame it on having small children in the home. I've been so scatter minded. That makes it SO much easier. Then in that case the ROS primarily applies when I see a new patient.
