A few general residency questions...

All three are program-dependent.

For me it was one week; yes; and July 1 (for orientation). I know people who had longer or shorter orientation, no pay for orientation, and not paid until August.
 
1. I know each program varies, but how long does pre-PGY1 orientation last? I've heard a week.

Varies widely from a few days to more than a week.

2. Do you get paid for orientation?

Varies. Some places do, some do not.

3. Generally speaking, when can you expect your first residency paycheck?

Also varies. For programs that pay once a month, you may not get a check until July 31st. If you get paid twice a month, your first check will usually be on the 16th but probably only for the first week of work (ie, the pay period will close on the 9th). Orientation may not appear in your check for several weeks. I was not paid for residency orientation but was for fellowship, but the check did not appear until the 3rd pay period.

There really is no way for you to know this in advance as every place does it differently.
