A question about "statement of need"

According to ECFMG, a "statement of need" is required to be elligible for J1 visa.
I have contacted the ministry of health in my country, and they told me that I must find a way to convince them that I will be back to my home country after the training in the United States. Otherwise they will not write this "statement of need" for me.
But I just can't imagine a way to convince them.
How do you manage to convince the ministry of health in your country? Do you need to mortgage anything? Or are you going to suffer any loss if you do not go back to your home country?

In my country we give a written undertaking to the effect that we will return after the duration of training. Also the health ministry in my country knows that doing your residency on a J1 visa mandates that you leave the US for a period of 2 years at least.

I suggest you give a notarized affidavit stating your intent to return. Also try to find out what other doctors from your country have been doing all these years. It's unlikely that you're the first IMG ever from your country.
