A question about the "Statement of Need" for J1 visa

According to ECFMG, a "statement of need" is required to be elligible for J1 visa.
I have contacted the ministry of health in my country, and they told me that I must find a way to convince them that I will be back to my home country after the training in the United States. Otherwise they will not write this "statement of need" for me.
But I just can't imagine a way to convince them.
How do you manage to convince the ministry of health in your country? Do you need to mortgage anything? Or are you going to suffer any loss if you do not go back to your home country?

Most ministries of health have a well-worn process for generating letters of need. Some countries have very easy processes. Others can be difficult -- Canada for example can be slow and difficult.

You and the ministry will need to find a way to come to some agreement. You could sign a contract with them stipulating that you'll come back after your residency. But it would be hard to enforce if you changed your mind.

Of course, I guess the first question is this: Do you plan to go back, or plan to stay in the US?
