What is the different between a fellow and an attending (I sort of know attending is higher up)? What does each of them do specifically and how long does it take to achieve those positions?

Well, I am not an expert by any means, but I think a fellow is somebody who is doing a fellowship in a certain field, for example cytopathology or pediatric anesthesiology. Attending is a physician in charge, somebody who is done with residency.
 
Fellowships come after residency training. Examples include cardiology after internal medicine, vascular surgery after general surgery, etc. An attending is someone who is done with training, i.e. someone who has finished residency and/or fellowship.
