Thanks to google, I found a template of the 2012 AADSAS application so you neurotic SDNers can have everything ready for June 1st (not that you need to.....). There are also instructions for each page, but some things have probably changed since 2012.

For the part that asks about family members who are dentists, do I have to include really distant relatives? Like a second cousin I've only met once and have no clue where or when he went to dental school? Thanks for all the help everyone

You don't have to include anybody if you don't want to.
