I'm putting the AADSAS evaluator information in...is this necessary? It keeps asking me for their street address...

Also...for "Dentistry Experience", do you know shadowing here? As a volunteer?
 
i put N/A for addresses. and you can put whichever, volunteer or shadowing. i put observer.
