when we fill out the courses that we took do we put both the letter grade and the number grade, example (4.0 and A) or just the number grade, example (4.0)?

Just the letter grade is all they require.
 
if thats the case then i am not sure what to put for a 3.5. each of my classes are different. a 3.5 is an A- in one class, but its a B+ in different class. are we suppose to guess?

An A- is a 3.66 and a B+ is a 3.333
 
is a 3.5 an A/B?? because it is between 3.66 and 3.33
 
so if my school only gives A = 4.0, B = 3.0, no +/- system, will my gpa be lowered on the AADSAS application, since school wont give A+, A, A-, etc.?
 
I think that they don't count the plus or minus. You either got an A or a B. Whatever your transcripts say is what you put.
