So, I picked D, but the answer key says the answer is A because once the voltage is turned off, the charge gradually leaks off. I understand that, but how were we supposed to know that the voltage isn't continually applied?

Are we just supposed to assume that a square-wave voltage means that there's a voltage applied and then it's turned off?

Yes. What else would it mean?
