Does it have a medical school attached? It's academic.
Is it not the primary hospital for the medical school but the students still rotate there? It's university affiliated.
No students or the only students are from caribbean schools or far away DO schools who don't have their own university hospital? Community or a very (exceedingly) weak university affiliation.
That's sufficient criteria for the vast majority of academic programs, though you may need to drill down a bit for something like the Hahvahd programs where there's (at least) 3 "primary" sets of residency programs affiliated with the school in varying combinations that are all academic.
