Can someone please explain how classes are filled? Does a school send out enough acceptances on dec 1st to fill its class? (assuming they have found students they want) If so why so many post dec 1 interviews? I know that some schools start the process later than others but ideally does a school try to fill seats right away? or are some left open for valuable but late applicants?

depends on the school. Some will send out more than their class cuz they know not everyone will accept. Some will take their class size and keep pulling off the waitlist till everyone is settled. Also some schools will only send out some acceptances for dec 1st (less than their full class) and then send out more later on. UCSF will only send half their class (44) on dec 1st.
 
I think it just depends on which school you are talking about.
Most schools fill only a portion of their available seats on Dec. 1st (some on Nov. 30 because Dec. 1st is Saturday this year). Then, it becomes rolling admission. This means every once in a while (every week, every other week, or every month depending on the school) the adcom will get together to discuss all the applications they received since their last meeting. Then, they will make more interview invites, acceptance offers, or rejections. This is probably why it's harder to get in later in the cycle - because you would have to really stand out from other applicants and adcoms have to make sure they are not "wasting" their very valuable seats.

The good thing is they most likely won't deny you on DEC 1st. They will hang onto you to see what the rest of the class is like, and then perhaps they will accept you later. I know lots of people who had interviews in October that didn't hear Dec 1st but got in and was notified of acceptance in like January or even later. But it all depends on the school.
