Access to pain fellowships from PM&R univ program vs Anes community program?

Say that there's 2 applicants for a pain fellowship, one from a university based well-recognized PM&R program, another from a community based anesthesiology program. Which one would have the upper hand, given similar research background, board scores, extra-curricular etc.

Now that pain is through the match, would being in anything else besides anesthesia put you at a significant disadvantage? How would the programs be able to pick just 1 PM&R/neuro/FM position out of the other anesthesia spots? Would they be able to rank EACH spot, as in if you had 4 positions:

1. Anesthesia rank 1-##
2. Anesthesia rank 1-##
3. Anesthesia rank 1-##
4. PM&R rank 1-##

or would it work just like the residency match where they scramble all fields together and rank them altogether?
