How accurate are the NBME's in predicting the actual thing?

I took NBME 4 five days ago and scored a 240. I was wondering, though, considering you might guess a good amount of answers correctly (which I'm sure I did)...how good is this at predicting the actual thing?

Hypothetically - couldn't someone guess the wrong answer on test day and go down 20 points?

I'm just nervous because my exam is in a few days and I'm beginning to second guess myself..especially since I'm forgetting things I went over a few days ago.

I found 4 most like the real test... but that does not mean 4 had any identical questions or very similar concepts of questions on the real test.

It was most like the real test in the sense that doing 1-3 had some clinical concepts not ordinarily covered in study materials, but would be something you would see on the wards if you stayed there long enough (e.g. management of death rattles)

However, there was nothing overtly "unfair" on Form 4, and I felt the same way on the real Step 2
