So, what’s the difference? ERAS has it as two “new” official fellowships. Addiction Psych is ACGME accredited via ABPN (psych&neuro), and Addiction Med is ACGME accredited via the American Board of Preventive Medicine aka ABPM. (Lol). You can apply to both coming from Psych, however!

I think addiction psychiatry is for psychiatrists only. Addiction medicine is open more broadly to other specialties. I suspect if you want to teach in an addiction psychiatry fellowship that the distinction might matter. I would be surprised if it mattered to anyone else.

W.r.t. jobs the fellowship doesn't matter if you are a psychiatrist. The hold-up is mainly that people who are not psychiatrists won't get picked for a job that's a lot of psychiatry (dual diagnosis, etc). Psychiatrist-run PPs will also prefer psychiatrists, as there's better cross-coverage for someone who's psychiatry (vs. other specialty trained).

Some IM/EM people doing AM fellowships are getting mainly addiction jobs (i.e. medical director at a treatment facility) and I think this will trend grow in size, though as you can imagine, the psych treatment is substandard and often outsourced (sometimes to an NP).

I think you will be able to get an addiction treatment job in almost any setting with both (or neither). I bet even an academic program could make any of it work, but no one outside of academia is going to care. They'll be happy to get a body.
