Address for sending PTCAS transcript

I just want to make sure that I'm entering the proper address for where my transcript needs to be sent. Is it the address on the bottom of the request form (PTCAS transcript department, P.O. Box 9112 Watertown, MA 02471)?

If that's the address on the form, then yes that's the one you send it to.
