Are there anyways I can see how the application form looks like? I wonder if there is the pdf version of the form or something like that?

If you submitted already, go to the Dental School Designations (middle column) section and click on the binoculars next to the school name (under Application PDF) and you should see your app as schools will see it.

I haven't applied yet. I am applying next year, and I want to see what items are on the application. Are there any way I can see those?
