Interested in Admin/Ops, have learned that there are ED-specific fellowships and then institution-level fellowships. Wondering which one could set me up for a bigger payday. I suspect Institution-level, but hoping someone out there may have some insight on which is more yielding financially.

Neither I guess.... But it depends on where you are taking your career. There is some component of compensation for admin time, but as a medical director of a level 1 trauma center with a residency program I can tell you it's more hours than the pay increase can cover for. Admittedly I have only my own experience. I have several friends in the community though, medical director, who say the same- the loss of personal and free time is not a one to one. Do this if you want it.
If your end goal is C suite, there are easier ways than the MD/DO route, but it can lead to higher leadership exposure and things like CMO. All depends on what you want....
I will say - do NOT do it with some grand vision of making money - it'll likely drive down your hourly rate if you are any good at it and give it that attention it deserves.
DO do it if you see yourself as a change agent, invested in your colleagues and your department, and feel comfortable finding the balance of metric whip cracking and simultaneously advocating for your colleagues and telling admin to shove it.... That balance is harder to find than you think lol

Yeah, nail on the head exactly in the bolded. I'm interested in flow, critical care in the ED and critical patient flows. Things like decreasing LWOBS, AMA etc. As interested as I am in departmental and hospital flows, as you point out, there is the issue of investing time vs. personal loss, and the yield per unit time. I don't know if I'm necessarily "a champion" who is totally fine with the financial losses. I already have an MBA from before residency.

Admin fellowships aren't necessary. I've never done one. None of our medical directors at any of our sites are admin fellowship trained. Many have their MBA's (usually online). I know plenty of admin fellowship trained ED docs who aren't doing admin.

How willing are you to sell your physician colleagues down the river?

My friend works for Envision in St Louis. He said he can interview you for their admin fellowship track. He also said to bring knee pads for some reason. Maybe he wants to go roller blading afterwards.
