admitting privileges -- active versus courtesy?

What is difference between active admitting privileges and courtesy admitting privileges? And, more specifically: Does one have to have active admitting privileges at one hospital in order to get courtesy admitting privileges at another? Or can you just apply for courtesy admitting privileges (without having active privileges anywhere) with the intention of admitting a nominal number of patients per year?
