Actually, being a vineyard worker is fairly unique.

You are extraordinarily strong in teaching which is of great value on a med school application. You'll have something to list for Artistic Endeavors, Hobbies, Leadership, and Research.

Your shadowing is about the average amount right now. I usually suggest aiming for 60-80 hours, and since you don't yet have experience with an office-based primary care doc, that's what I'd suggest to round out your experience there.

You will need a regular weekly position in a medical environment, be it at a hospital, clinic, nursing home, hospice, or rehab facility, etc. Most have about 1.5 years of clinical experience when they apply. The pharmacy internship will help you there and will count toward that expectation during the time you are interacting with patients/counseling on medication. Even if patient contact is minimal, a knowledge of pharmaceuticals is certainly relevant to your application. If you find you aren't getting enough patient contact, perhaps you can add further time in some other part of the clinic once they know you.

During your off year, at a minimum you'd want to continue some nonmedical volunteering as well as some type of clinical experience, whether job or volunteer. Or you could work in a lab. Or you could take any employment as long as you continue volunteering for 3-4 hours per week and get patient contact for the sake of update letters and interview conversations.

I hope you give a good description of working in the vineyard on your application, because adcomms will want to know all about the necessary tasks. Make it exciting.
