When writing thank you notes, is it appropriate to tell program directors that their residency is your top choice or you plan on ranking them highly? Or do you have to kind of beat around the bush? Thanks!

Not really appropriate or necessary. You CAN say I am reallly excited about your program and thank you for the opportunity to be considered!

The thank you note template I have is from a medical interview prep course, and it does state that "I am planning to rank your program highly". I think this only shows your interested and impressed, it doesn't necessarily mean you're ranking them #1. And I only use this to programs that I am truly considering ranking highly.

I don't think it is appropriate to state about ranking at all. Saying so only would reveal that one is not sincere or honest since one has not even had basis of comparison if you are going from one interview to the next. I look at it this way. Matched at a specific program or not, all these people will be your future colleagues and it is not a bad idea to start networking early on. I also heard that most programs don't come up with their rank list right after interview. When the admission is so competitive all the little things count.
