I don't know what I'm missing, but it looks like the FMG has to have an ECFMG certificate, and do 4 years of anesthesiology in the US. So what is the benefit or difference from the regular pathway for foreign grads?

Even if the person does the "expert" role and stays at an academic place (like when I was a resident (in another field), there were some foreign anesthesiologists who only worked at "the big house", and were allowed to, by law, even though they hadn't done US anesthesiology training), it's still 4 years.
