Grades are 100% my biggest concern. I did horrible my first few semesters before deciding to go to medical school, so my initial GPA was terrible. I've had to work full-time (40-45 hrs/wk) throughout all of college, which definitely hurt my studying and grades, but I was able to pull through with about a 3.5 average for my last couple years.

PLEASE PLEASE PLEASE let me know if its a good idea to apply this upcoming cycle or if I need to do something else/more before applying. Feel free to comment or send a DM. Thanks!!

Can you explain why you were both working and ramping up your college course hours? You get no bonus points for overloads.
I had to work to cover life expenses for both my wife and I. Tuition, huge medical bills, rent, etc. (parents didn't help and financial aid only covers so much). I also took as many course credits as possible to turn around my terrible freshman and sophomore GPA which is what my advisor suggested I do to repair my GPA.

You would benefit from a 1 year DIY post bacc at a local college. Take undergraduate level science courses in order to increase your sGPA. At this time UTAH is your only realistic MD school. You are competitive for many DO schools though some have a sGPA cutoff of 3.2

Your sGPA went down again to 3.18 in your 40 credits your senior year. It does not really matter then if you did well in non-science classes during that time. I do not think MD schools are realistic, including your own state school. Retaking Ochem and getting a C the second time does not help either.
