Am I Screwed?

I was hoping to get advice as to what type of medicine programs I should apply to?

I really want to go to a good academic medicine program but I am wondering if I am screwed?

I did honor in medicine and my grades I guess are average but I did not get honors in everything mostly because of the shelf so I do have really great clinical evals and my deans letter and LORs will be great.

I have tons of extracurriculars and lots of research though no publications.
