What kinds of programs have you seen pharmacists involved with in Ambulatory Care type settings?

I have met several pharmacists involved with coumadin clinics, diabetes management programs and a lipid management program. Those are the more common ones that I have heard of. Have you guys seen pharmacists involved in other kinds of disease management programs in the clinic-setting?

they mainly do cardio and diabetes, but I'm pretty sure they have expanded their services with MTM so they may offer more now.
