Hi All,
Wondering if anyone has come across any specific recommendations when switching from amiodarone to dronedarone. The issue is that you do not want to double up on anti-arrhythmics because of the potential for Torsades and QT prolongation. Normally you would hold a drug for around 3 half-lives before initiating the new therapy, but this becomes an issue because of amiodarone's huge half-life (up to 55 days!). A search through Medline didn't have much and neither did the manufacturer. Any thoughts or experiences with this would be appreciated. Thanks so much!
