Since S. pnuemoniae is the most common cause of otitis media and you can treat otitis media with amoxicillin, but why is it in First Aid, they don't emphasize that amoxicillin can kill s. pneumoniae?

I think its meant to be what penicillin covers + what they list. Its just listed like that to be less redundant.
 
S.pneumoniae OM and meningitis you would use a beta lactam.

For OM in real life, they usually use Augmentin (amoxicillin + clavualanate)

Recall for lobar pneumococcal pneumonia, macrolides are in order.

not quite sure. maybe a pharmacologist can answer that better.

my guess would be drug resistance.
 
okay.. so this is a case of just memorizing rather than knowing the mech or whatnot.

cool. i just wanted to know if there was an underlying reason. makes things easier than rote memorization, not that rote memorization is bad.

so, you can actually use Penicillin for pneumococcal infections. There're a couple of reasons why nobody uses it (aside from for GAS pharyngitis); one, you rarely actually have a culture proving it's s pneumonia, so you want to cover other bugs like H flu, M cat, etc. Two, and most relevant in peds, penicillin tastes bad and amoxicillin tastes better. Three, docs are creatures of habit and they just don't use straight up penicillin anymore, w/ a few exceptions.

In addition first line treatment for AOM is amoxicillin high dose 80-90 mg/kg, NOT augmentin. Augmentin is then for failure of first line treatment.
