So I'm working on the primary application still, but I want to get my LOR's ready. Do the schools have any specific evaluation forms or specific questions they want answered by the LOR writers? I don't want to have to go back and tell them to rewrite the letters.

Most schools will accept a "generic" LOR. Information the writer can include:

- how they know you
- how they are able to evaluate your desire to become a physician
- what do they see in you that reflects positively
 
i think the only school i've come across that requires you use their form is michigan state
