Would do you think of the following approach: describing multiple failures you've faced in the past, and collectively describing how you grew?
No single failure stands out to me. They're all important in establishing values like persistence in the face of adversity...
 
Personally, I would choose one challenge and expand upon that particular problem. That way you can express the lessons you've learned and develop it a little better.

Following directions is important, stick to the prompt. Pick one and elaborate. Find a way to make it stand out.

It doesn't have to be about overcoming failures. Any challenge you faced! Whether it's obstacles in life, things you had to adjust to, etc.
It can even be as simple as getting your first job, or working in a team-based environment when you're a total loner. Or working with a mentally ill patient, or communicating with ESL people.

Following directions is important. However, the way the prompt is worded, a 'problem' can be multiple failures culminating in a single centralized issue that was worked on. If they aren't connected, I would agree that you need to focus on one of them.
