Anthem Provider Termination... success???

Has anyone had any success terminating from Anthem as a provider... we have been trying for 3 months now without success. Is this typical? Anyone else had problems terminating from Anthem as a provider?

There should be an address for notices in your Anthem contract. Send them a certified letter, return receipt requested, terminating your agreement with the effective date based on your termination without cause notice period. You generally have to give 90-180 days notice but depends on your specific contract language.
