Well,I think it would be difficult to land a spot in scramble with visa requirement especially H1.
Scramble is a desperate situation for both the programs and the applicants.
Programs would prefer taking a candidate who comes with lesser hassles(you know what I mean).I wouldn't say its entirely impossible though,but I think transitional year wouldn't be an easy catch,prelim surgery should be the easiest.
On the other hand if you have an advanced position for anesthesiology ,it should be relatively easy for you especially in those community programs(prelim medicine) which take IMGs.
