We are learning about ointment, creams, gels in class. I don't fully understand the concepts, I have tried to research but I kind of a need a simple explanation. I understand what emulsions are but in terms of creams, ointments, and gels- I do not, how are those emulsions ?. Why is it that the BASE YOU USE has a tremendous impact on how the drug is released. I am confused on the 5 types of bases: Oleaginous Ointment Bases, Absorption Ointment Bases, Water/Oil Emulsion Ointment Bases, Oil/Water Emulsion Ointment Bases, Water-miscible Ointment Bases

WHICH DRUG CAN BE INCORPORATED INSIDE THEM ? For each one, Hydrophobic or Hydrophilic ? I just need to know the REASONING BEHIND IT. And How that will impact the release of the drug ?

CAN SOMEONE PLEASE GIVE EXAMPLE OF WHEN YOU WOULD USE OINTMENT OVER CREAM In terms of pharmaceutics reasoning ?

What would penetrate the skin better? Hydrophobic or hydrophilic? Do you want the medication to remain in the skin or do you want systemic absorption? Think about it for a second...

I could never really remember them until I saw/felt the examples. If you don't have lab yet, check out your compounding section at work. It will make a lot more sense to see the items.

The difference between ointments and creams is that a cream is an emulsion, readily absorbed by the skin. If you have an acute case of eczema for example, using a cream form of an anti-inflammatory drug is better because of its rapid absorption and hence fast onset.

Ointment stays longer on the skin, so has slow onset but prolonged action. If you have a chronic case of eczema, an ointment form of an anti-inflammatory drug is preferred because the ointment will stay longer, and will form an occlusive layer on the skin so increases the skin moisture, which is required considering that dryness is a symptom of eczema.

The difference between ointment bases is mainly their ability to absorb water and their tendency to be washed off by water.

A gel form does not include any fatty esters. This is can be important in the topical treatment of acne for example, since acne is caused by the fatty acids that are secreted by the sebaceous glands and then clog the skin pores. That is why retin A gel of benzoyl peroxide gel are favored for topical treatment of acne vulgaris.
