Anyone describe a neuro/cognitive function test

What tests do neurologists use to assess damage from concussions in sports? In essence, what deems a player able to return to action in a sport, & what would cause a player to fail a test & not be cleared for play? Is it motor function or mental function only? Anyone give a brief explain of what they entail?
