I can only speak for KCUMB since that is where I will be going and I have also considered the DO/MBA program. You apply by the Nov. of your 1st year. Then I think you start classes during the summer between MSI and MSII. You will have to look at the website for the exact curriculum/class schedule. There are many pro's and con's to the degree. Just briefly:
Pros:
Just about any education is good
Easier to get MBA as a student than when you have a family/own practice
Can get it in a shorter time than usual
Make some good contacts?
Cons:
Cost
Loose your free time (ie. your summers or have to take 5yrs)
Depending on what you go into, the usefulness (how much will you use it)
Not all programs are equally strong

I am also slightly considering DO/MBA program. You don't need to apply for the dual program right away. I believe you can start if after your first year. I do hear PCOM has a great program, but not sure about the rest. You can also look for MBA programs that colleges/universities near by offer. Some are for working professionals and offer classes on weekends (ie UCLA). Good luck!
To be honest I don't really know. I think you would have to look at the University giving the MBA and see what their reputation is. I know that Rockhurst University (KCUMB) has a fairly good business school that is respected in the midewst. But outside the region, I am not sure. I know PCOM seems to do most things well, so I am sure their program is solid as well.
