APGAR: Crying with and without stimulation

For the APGAR score, if the child is crying when born but only grimaces when stimulated, does that mean that the reflex irritability score is 1 OR does the respiratory effort evidenced by (unstimulated) crying automatically give the child a 2 for the reflex irritability?

I'd go with 1.

A cry does not equal a 2 for respiratory effort either. A strong cry gives you a 2 for respiratory effort but a weak cry can give a 1.

Grimace is all about what happens with stimulation. The stimulation is usually either on the sole's of the feet or inserting a tube into the neonate's nose. Only grimace then you get 1. Grimace plus STRONG cry, cough, sneeze etc is a 2.
 
I picked 1 based on the same reasoning you mentioned but the right answer was choice 2. I am pretty sure it is an error then.
