Hello everyone, there is a portion in the "Self-Query Report" (which is a new requirement as part of the Pharm Tech License application) website https://www.npdb-hipdb.hrsa.gov/ It's asking for your state license number and a ticker option you can check, that says "No License"
Basically, my state license lapsed and I had to apply again as a "new" licensee, BUT I already have a license number assigned to me that is still in the gov board website although the status says "Cancelled"
The 800 number in npdb is not helping and don't know the proper thing to do either. It states in the application that you have to be very precise with the steps or else it will delay the application again (avg 2 months)
Should I put my license number? Or check the "No license" option since the status is currently "Cancelled"?
