For each specialty you apply to, you need an essay and 3 (three) letters of recommendation. There's an on-line recommendation, but some schools request that the letters be sent directly to them. Not only that, some programs have a secondary application and fee for which you'll need to go to the school's website, or contact them if you can't find out the information from the web. You'll also need a dean's letter - which is an on-line form filled out by the dean's office which states your class rank and board scores.
Bottom line is that each school you apply to will have different items that they request as specified by the PASS website. (Go to the program designation search engine here:
http://www.adea.org/EPS/SearchForm.asp). Don't go off of this yet, it may change for next year.
To ease this burden, request your evaluators write letters separately, and give them stamped addressed envelopes for each program, and give the envelopes to the dean's office as well. Also, you may want to prepare cover letters with a 2x2 photo to attach to them. Many programs request transcripts from undergrad schools and some want your dental transcripts too. In addition, there may be a need to send your board scores directly to certain places (look at the search engine). It's a basic full-time job to organize the apps and track down professors about your recommendations. Even when you do everything on your part to ensure completeness, the programs still might not get all of your materials. This happened to me as PASS sent my online letters for 1 (one) evaluator without sending the others because of a computer glitch that allows you to designate programs with just one evaluation being sent. I hope they iron that one out for this year's applicants (2007).
One more important detail is obtaining your match number. Around the same time that the PASS application becomes available (Aprilish), you need to buy a match number from this website:

It was $40 per program plus an original fee of ~$175 for this year's cycle through PASS. It also costs money to get a match number, but I can't remember how much I spent.
