Is it bad to apply to two different specialties in the same university, but in different cities in that state? For example applied for anesthesiology in school X in city y, and internal medicine(backup) in same school X, but city Z. Both invited me to interview, and I am worried if this might be a problem.

Most likely it won't come up but you should be prepared for the possibility it will come up and be prepared to explain why you are applying for two specialties. Kind of a hope for the best and prepare for the worst kind of approach.

Residents who have matched in comeptitive fields like Optho, Rads, and Uro have all said the same thing. DO NOT have a backup. If you are comfortable with the backup, then you can do that, not their specialty.

I disagree, because you want to match, not scramble.

However, that advice DOES hold true for programs. Do not apply to multiple specialties at one hospital.

exceptions to this are:
(1) Prelims
(2) Reason to stay in the city, i.e. you dont care what you do but you must be in the city for a spouse, family member, friend whatever
(3) You have a screaming desire to do what most people would consider bad taste just to prove them wrong.

Good Luck.
