Applying for the DAT test, do I have to fill out the following section? (quick question)

I am on the fourth or third page of the DAT applying process and its asking all these personal questions about my background. Am I required to fill in this information or is it optional? The page tells me to do the following:

Please fill in data requested below for the DAT program. The information you provide will not be communicated to any persons or institutions without your permission. The data will be used solely for studies regarding test validity and group performance. If you are not able to enter exact figures, please make your best estimate.​

If anyone with experience would help me out it would be quite appreciated.

Does anyone know the answer to this? I submitted one without answering them after my high school information and now I am super worried...
