Applying out from a combined-degree program

For a combined degree (BS/MD or BA/MD) program in which you lose your spot once you attempt to apply out, I was curious, how does the school know that you are applying out? AMCAS claims that they don't inform one school about others until sometime in April. If this is the case, wouldn't the bs/ba/md program find out that you are applying out only after you receive decisions from other schools?
 
Sure, I suppose you could do this. You'd only need to forge all your prof's signatures on the letters of rec/committee letter... In all seriousness, though, it'd be dishonest to try and pull the wool over your school and I doubt you'd do very well when your school blacklisted you for telling them you intend to stay in their program and also applied. At some point, they'd find out, and when they did...well, you'd find out just how fast news travels in the medical academic world. *snaps fingers* My guess is you may not want to burn any bridges just yet.
